"taking heed" means to build a surrounding hedge of thorns, to guard or protect, to look carefully at, to pay attention to.
How extreme should you get to protect yourself? See Matt 5:30.
This verse says build a wall in front of anything that might cause you to sin according to God's Word.
In the areas in which you struggle, what type of walls can you build to keep you away from temptation? How about not going there, or not buying something, or not watching something, etc.?
Have you built these walls? How have you built them? How easily are they removed or compromised?
This verse also implies taking the time to think about this, rather than continuing without a thought.
