Without the guidance of the Holy Spirit, they cannot understand - it is a dark, foggy cloud to them.
There are many people who pretend to have a relationship with God - they talk about prayer, about heaven, about God, on occasion, but they are alienated (separated) from a life with God. And, they are blind to this - they think they have something when they have nothing.
The sad thing is they are alienated from God because of their ignorance, of simply a lack of knowledge. You would think that you could simply educate them, but it's more difficult than that, because it is also a matter of their heart, that their heart cannot see or understand or desire the things of God, because their heart cannot see them - their heart is blind.
And this blindness is hard to affect because of the hardness of the heart - it is past feeling (vs 19).
The result of this ignorance, blindness, hardness is unrestrained sin, especially regarding sexuality and money (vs 19).
