The sheep were kept in an area enclosed by walls, which had one door to let the sheep in and out of. The doorkeeper controlled who was allowed to bring sheep in, and who could come and collect their sheep.
The one who comes in through the front door is one who is known (verse 3) and has the authority to do so. See notes on Psalm 23:1.
