The Pharisees supported themselves in their opposition to Christ with this principle, that they were the pastors of the church, and that Jesus, having no commission from them, was an intruder and an impostor, and therefore the people were bound in duty to stick to them, against him. In opposition to this, Christ here describes who were the false and true shepherds, leaving them to infer what they were.
"Most assuredly" is also translated as "Truly, truly", "I tell you the truth", "Verily, verily". In Greek, it is actually "Amen, amen.", the same word used to close the Lord's Prayer in Matt 6:13. But here it is a strong emphasis on the certainty of what is about to be said. This is similar to when we say "Amen" to show strong agreement with a statement.
The form of speech that Jesus uses here is a similitude, which are used to illustrate and simplify divine truths.
List things that get you going, that lights a fire in you, that motivates you. Notice the extremes that those who devise evil are willing to take to succeed in their plans, here climbing over the wall. This should shame us out of our slothfulness and cowardice in the service of God. Those who devise mischief are willing to climb the walls to accomplish their desires, but we are unwilling to walk through the front door. God has asked you to do certain things, but what will it take for you to do them?
The thief and robber is anyone who tries to take unrightful ownership of the sheep, which is anyone who draws them away from the True Shepherd. Anyone who teaches a false religion or false gospel is a thief or robber.
