And the city shall be accursed, even it, and all that are therein, to the LORD: only Rahab the harlot shall live, she and all that are with her in the house, because she hid the messengers that we sent.

Why kill all the inhabitants? That Israel would not learn to follow their false gods -- Deut 20:17-18.
Were the Israelites reluctant to do this? God wanted them to get rid of anything that stood in the way of obedience and worship to Him. What stands in the way of your obedience to Him? Has He asked you to do something that you are not willing to do? Is there something you need to change that stands in the way of obedience to Him?
They could have chosen to not obey, and the Israeli society would have been much weaker. So too when you choose not to do what He is asking you to do.
Anything causing you to sin must be removed -- Matt 5:30 (even your right hand). But this also includes places you go to, people you hang out with, things you read or watch, where you work, etc. God probably won't ask you to cut off your right hand, but He certainly might want something out of your life if it is causing you to sin.
