Revelation 20:15

The lake of fire is eternal separation from God, and is eternal torment (vs 20:10).
Those thrown into the lake of fire was not based on their deeds, but whether they were Christians, whether they trusted their life to Christ, i.e. they were in the Book of Life. Without Christ, their life is judged, and they are found guilty (Rom 3:23, James 2:10).
